Accident investigation and lessons learned
A near miss analysed is an accident avoided. Fact gathering, causal tree, action plan, follow-up of measures: the method turns an isolated event into a lasting improvement of the prevention system.
Working back to the deep causes
A useful investigation looks for the system rather than the culprit. The causal tree conducts that work rigorously: start from the observed fact, work back chain by chain, separate what triggered from what allowed. What emerges is nearly always organisational — a deadline too tight, a tool missing, an instruction that became impractical. The programme teaches that method along with the arrangement that keeps it continuous: near-miss reporting, indicators and periodic review.

How is the effect of the actions measured?
Through steering indicators defined in the certificate: reporting rate, time to resolution and on-site verification of measures declared complete.
